Completing and submitting a new application

By when does a new membership application need to be submitted?

Application forms need to be submitted before or on the 1st of every month, in order for the membership to be activated from the beginning of that month

What supporting documentation needs to accompany a new application?

A certificate of membership from the previous medical scheme must be attached to the application form

  • Should the previous scheme no longer exist, a detailed declaration of previous medical aid cover must be submitted with the application form
  • Child/Student dependants who are over 21 years of age, and up to 27 years of age and are either full time students or financially dependent on their parents must either submit a student certificate (current year of study), or a sworn affidavit stating the child is financially dependent, residing with the principal member, and not earning more than a State pension
  • Adopted/ Foster – Copies of legal documents proving guardianship must be submitted with the application form
  • Grandchildren – Sworn affidavit stating the financial status and whereabouts of both the child’s parents and the details of the circumstances need to be submitted with the application form
  • Parents – who are residing with, and are financially dependent on the principal member and not earning more than a State pension. An affidavit to this effect needs to be submitted with the application form, as well as supporting documentation where applicable
  • Special or permanently disabled dependants – a medical report needs to be submitted with full details disclosed to the Scheme

What information needs to be provided in section 1?

An option needs to be selected and the commencement date needs to be completed

What information is required in section 2 –  Details of principal member?

Main member details – all fields must be completed, including ID number

  • Full names are required
  • Full street and postal addresses are required
  • Cell phone numbers are required, as Fedhealth will use this information to advise members of their medical aid number once registered, via SMS, as well as the status of their membership and claims
  • Members’ email addresses will be used to send electronic statements, claims staus updates and other Scheme communication
  • Certificates of previous medical aid cover must be attached to avoid unnecessary underwriting being applied. Should this not be completed correctly, waiting periods and Late Joiner Penalties could result

What information is required in Section 3 – Broker details?

The broker will be required to complete this section in full.  Remember to include your Fedhealth broker code as failure to do so could result in commission not being paid correctly!.

What information is required in Section 4 – Details of spouse/ partner ?

  • Full details of spouse/partner, including ID number must be completed in this section
  • Full names of the dependant are required
  • Certificates of previous medical aid cover must be attached to avoid unnecessary underwriting being applied. Should this not be completed correctly, waiting periods and Late Joiner Penalties could result

What information is required in Section 5 – Dependants?

All dependants, including children, must have their ID numbers completed. NB: child dependant rates will be deducted until the child is 21 years of age

If over 21, child rates will still apply until the age of 27 based on the following:

  • The child is unemployed, is financially dependent on the main member, and living at home, or not earning an income more than a State pension. An affidavit will be required to be submitted with the application form stating the above
  • or, if a full time student, proof of the current year’s study will be required in the form of a certificate of study from a registered educational institution, to be submitted with the application form
  • Dependants other than the member’s children, such as adult dependant parents should also be listed in this section, and the supporting proof of dependency in the form of an affidavit, must be submitted with the application form

What information is required in Section 6  – Employer Information if contributions are paid by the employer?

  • Employer to complete
  • Affix the company stamp
  • Ensure that the paypoint code is completed
  • HR officer to sign and date
  • The employment date of the member must reflect under this section, as this will determine the underwriting imposed on the member

What information is required in Section 7 – Medical Details?

This is a mandatory section for a member to complete:

  • Please complete the yes/no question
  • If yes, provide all medical details relating to the condition, medication and dosages necessary
  • Note: pregnancy must also be included in this section
  • Special care when detailing the diagnosis needs to be taken, e.g. knee injury if not sufficient information – left or right knee as well as the detail of the injury, surgery or fracture
  • Non – disclosure of treatment, medical advice, diagnosis or care of any condition for any dependent will result in termination of membership
  • Treatment after the application form is signed – the Scheme requires full details of treatment, medical advice, diagnosis or care of any condition for any dependant that occurs after the application form has been signed and before the membership is activated. A medical report will be required

What information is required to complete Section 8 – Banking Details?

  • If the employer pays the contributions over to the medical scheme, the member needs to only complete their banking details on the right hand side “for claims and savings refunds”
  • Full banking details are required. Should the bank account NOT be the member’s – the account holder is required to sign the application. The account holder also is required to supply a signed letter of authority stating the Scheme may debit his/ her account for the contributions, as well as supply proof of the bank account details and a copy of their identity document. (Internet statements are NOT proof of banking detail and will not accepted)
  • Members/ account holders are required to sign and date this section

What information is required to complete Section 9 – Declaration of principal member?

Members are required to familiarise themselves with the content of the declaration, as well as sign, date and complete their ID numbers on the declaration
